I work on 12 reqt projects quite often these days. Doing it at the moment - 40-50 hours estimated. Some examples: Make user interface much more friendly. Ensure that historical data is not lost. Include RCD testing - similar to T&T testing.
They need a lot of trust both ways, and a lot of understanding of the user domain by the developer, ie the developer needs serious RE skills.
OTOH, I've worked on projects that had 1000s of requirements. On one, there were about 500 *user* requirements but 20,000 (mostly derived) requirements total at the end.
As a rule of thumb, I normally wouldn't use an RM tool for fewer than (say) 400 requirements total. I've found that we can normally handle that many manually (it takes about a day to trace them in any direction). I also wouldn't put the (user) requirements normally into an RM tool until after initial analysis - perhaps the 3rd draft for review. They are too immature, particularly in structure. I've found that using RM tools tends to inhibit certain types of sweeping changes that may be necessary, and which would have a big positive effect on the remainder of the project.
