I do not know of anyone who is willing to let their requirements documents out of the house. All of the ones that I have are covered by non-disclosure agreements and my own sense of client loyalty.
However, our requirements specification template might be of use to you. You can download this at http://www.systemsguild.com just follow the links to Template. There are also some other resources there that may be of use.
